To make ps2pdf use a specific page size use the following options:

-dDEVICEWIDTHPOINTS=w -dDEVICEHEIGHTPOINTS=h -dFIXEDMEDIA
where w is the paper width in 1/72″ and h is the higth in 1/72″ units.
